Wiki Medicare E-Visits for Physical Therapy for Texas

I have been trying to get our E-visits paid my Medicare but they continue to deny. I have billed every way a Medicare rep has told me:
with GP/95 modifier (this was also on their website), only 95 modifier/ without any modifiers. The POS is 11 which is correct.
Has anyone billed these and gotten paid. If so, would you be willing to share with me what modifier (s) you billed?
