Hi all,
I need some clarification on the following:
Medial branch nerves: C4,C5,C6(3 branches) is equals to 64633,64634,64634-59?

And

Facet joint injections L4-L5, L5-S1(2 levels) are equal to 64493,64494?

Your help is greatly appreciated:)
 
You are correct with the facet joint injections (64493/64494), but make sure to use laterality modifiers. We've found that using mod -50 for bilateral works, but we have to change the amount charged, so I'm not sure how that would work for you.

64633 and 64634 are for destroying nerves using radio frequency, so I'm not sure if that would work for you. I don't have my coding book (we use CODEX), so I'm not sure if that is really what you're looking for since I'm not 100% sure if that includes the medial branch nerves. I can tell you that those codes would work for those levels.
