One of our providers had a patient come into the office to establish care. The patient has a traumatic brain injury from an auto accisdent in 2005. He also has several other problems all related to the accident. The mother of this patient had all kinds of scripts from other providers that were never done and wanted our provider to issue other scripts and order labs. The provider performed an HPI, ROS, PFSH, Exam. Her assessment was to refer patient to a Neurologist, Urologist, and ordered labs. The provider coded this as a New patient 99204. The provider also feels that this should be sent to the patient's medical insurance but the mother of the patient is all upset because she feels this should be billed to the auto policy. Please give me your thoughts on this. Thanks for your help.
 
Our providers would documente two seperate notes. One for the brain injury to send off to the auto carrier and another note for all other issues to be billed to the health carrier.
