Wiki Medicaid billing for wwe

Hello all! I am not familiar with Medicaid billing and codes required for billing on a Well woman exam. I received a call from a former student stating her claims are being rejected by Medicaid and certain Medicaid managed care plans with ICD code V72.31 and the age appropriate E/M wellness code. Please help! Where can we find this information? Or would you please be so kind as to share the correct codes. Thank you!

it is possible they want the HCPC II codes for the well woman same as Medicare, or the timing is off. if it is not one of those two things I am not certain as to what the problem could be. Medicaid is different in each state.
