We have multiple programs under the umbrella of our agency. One of our programs working with teens and pregnant mothers would like to bill for a smoking cessation program we are trying to put into place. Codes 99406 and 99407. How do we bill Medicaid, how often can we bill for an individual, what is the type of bill we would use, place of service, etc.
