I have been having a little back and forth about coding cervical median branch blocks.
For example, a bilateral C2, C3 median branch block. Per AMA 2024 CPT book, this would be coded as 64490-50.
I'm being told to code this as 64490-RT and 64490-LT. I can't fathom how that would be right??

Now I have noticed in some instances they will put bilateral C2 AND C3 median branch block.
In this instance, would you code 64490-50 and 64491-50 because the verbiage is different?

I know MNBB's are billed by the level and not the nerves, so it's considered only one level. I'm not sure if I'm just overthinking or what.

Any help or insight would be appreciated!!!
 
64490 is the correct code for medial branch block. Whether you use modifier 50 or the LT & RT may depend on the payer and what they want. I code for hospital outpatient and I always the -50.

I'm an ASC, so from what I have found researching is that an ASC can bill 64490-RT and 64490-LT.
I just don't want to start causing issues and insurance claims get denied.
