We have some carriers denying our 3rd level i cannot find anywhere that the guidelines changed , other than for the bilateral guidelines. They are denying our 3rd level . We have normally always coded it like this....
64635-RT
64636-RT
64636-76-RT

should we be billing it
64635-RT
64636X2 RT ??
Any input would be greatly appreciated .
