I have a provider that is documenting time spent during a telehealth visit as 10-15 minutes (99212); however, their medical decision making that is documented would support a 99213 or 99214. If they have documented time spent do we have to code the E/M based on time or are we able to disregard the time and code based on their medical decision making?

You may code on medical decision making OR time. Chose whichever is more beneficial for you in each circumstance.
