In MDM table C in moderate Risk column the sentence "Undiagnosed new problem with uncertain prognosis" what does exactly means and when can we take moderate risk for giving ED level kindly suggest with some examples with reference materials.

For Example: 1.If a patient present with chest pain and MD took EKG and that was normal and patient discharged this case we can consider as Undiagnosed new problem with uncertain prognosis ?
2.If a patient present with abdominal pain after taking labs MD cannot able to find what exactly the condition was and suggest the patient to consult respective specialist, this case we can consider as Undiagnosed new problem with uncertain prognosis ?

Thanks,
Manikandan, CPC.

I reserve using "Undiagnosed new problem with uncertain prognosis" for the provider who will be formulating a diagnosis after reviewing ordered test results.

So I would not use it for either of your examples.
