Wiki MDM for declined surgical procedure

Provider has a lengthy conversation with a patient discussing surgery. He went over risks and benefits, etc... Is the level MDM he chooses affected by whether or not the patient agrees to have the procedure? At the end of the conversation, she declined.

None of this matters for MDM. For MDM identifying the need for surgery is factored in. It doesn't matter if the patient chooses to have the surgery or not, the need for it has been identified.
