In the guidelines it states "Note that to qualify for a given type of medical decision making, two of the three elements must either be met or exceeded." My interpretation is that all 3 need to be documented but only two of the three elements must meet or exceed a level to determine the type of decision making.

I have also received another interpretation that only 2 of the 3 need to be documented in order to determine the type of decision making.

Please help with this. Thank you!!
 
You would not always have all three elements. Such as the provider may not have anything that would apply to the data reviewed such as:

* Labs ordered or reviewed
* Xray ordered or reviewed
* Medicine ordered
* etc.

So in this case you would only have the diagnosis and risk factor to make the determination.
