We have a dilemma as to how many levels were injected. I would appreciate a reply and rationale please, Thank you,

BILATERAL L3, L4 FACET MEDIAN NERVE BRANCH BLOCK AND L5 DORSAL RAMUS NEVER BLOCK; TOTAL NEEDLES 6

It sounds like three levels, bilaterally, but there isn't enough here to say for sure - can you post a complete procedure note?
