The patient's right hand tended to have swan neck deformities, or stiffness, in the metacarpophalangeal and proximal interphalangeal joints with intrinsic tightness. The index, long, ring, fifth finger each in turn was manipulated. The thumb interphalangeal joint was also manipulated. The metacarpophalangeal joints were placed into 90 degrees of flexion, and then the interphalangeal joints were flexed to 90 degrees of the proximal interphalangeal joint, and 60 degrees of the distal interphalangeal joint. The metacarpophalangeal joints were then extended and the proximal interphalangeal joints were flexed to 100 degrees of flexion in the index, long, and fifth fingers The metacarpophalangeal joints of the index, long, and fifth finger, and the proximal interphalangeal joints of the index, long, ring, and fifth fingers, and interphalangeal joint of the right thumb were then all injected with 0.5 cc of Marcaine 0.5% and 0.5 cc of Celestone Soluspan. A sterile compressive dressing was placed on the hand. The IV block was released, and the patient was taken to the recovery room in good condition.
