If a patient with previous diagnosis of bladder cancer comes in for a cystoscopy and there are no new tumors found, should you report this as a history of bladder cancer? At what point do you code it as a history? Thank You

It is a hx of when there is no evidence of disease and there is no treatment being directed toward the neoplasm
