Just starting to bill fracture mgmt codes. Question is should you bill E/M code on initial visit with fracture mgmnt services w/modifier 25 appended?

Not unless the patient is being seen for something other than the fracture. For example, if the patient has a fracture, but is also being seen for uncontrolled diabetes and hypertension, you can bill for the office visit with the appropriate modifier, but the provider needs to be sure to document what amangement was done for the diabetes and hypertension.
