I have a provider who performed a planned cesarean section, but while she was in there, she had to spend an additional 45 minutes for lysis of adhesions. Does anyone know how I could bill for the additional time/effort? My original thought was to bill the 59510 with 58662, but when I look in the OBGYN coder specialty guide, it doesn't show that I can code them together.

58662 Laparoscopy, surgical; with fulguration or excision of lesions of the ovary, pelvic viscera, or peritoneal surface by any method
This is a laparoscopic code and it is not for lysis of adhesions. Double check the CPT for open lysis of adhesions and then see if that is an NCCI bundling edit.
If there is additional time/effort and no more appropriate code to use, then consider modifier -22.
