I have a path report that states this dx. here are additional notes:

The sections show acanthotic epidermis with parakeratosis.
There is superficial and deep perivascular lymphohistiocytic
infiltrate. Focal granulomas are identified. The collagen
bundles show focal degenerative change.

I would go with 686.9. In this case, "lymphogranulamatous" is an adjective describing the type of inflammation.
