I have used the V-codes especially for the use of asprin and if they are on any other meds that are over the counter such as anti-allergy etc.. Even if it is from the pt's list from home. If a copy is on the chart it's considered appropriate documentation that you can code off of as long as it's filed correctly within the medical record (dated and signed by Dr or staff as reviewed with the pt)

I would use the date that the Dr signed it. If he reveiwed the list say two weeks later with the pt and there are no changes then he would need to document that in his note that he reviewed the meds with the pt and there have been no changes. Obviously if there are changes then he need to document that also.
