We have a medical director (MD) who has been deployed overseas. We have a locum physician who is covering for him and we bill this as a locum with the Q6 modifier. Recently, this locum physician was covering for the NP one day and saw some of the NPs patients. How do we bill this?

The NP was out for the day and the physician only saw her patients that day. Could this be billed as a locum since normally the medical director could cover for the NP one day and see her patients for the day if he was there?

Thoughts?

I would bill this under the medical director info with the Q6 since the locum was doing what the doctor normally would have done had they been there.

A doctor can not be a locum for an NP so when your medical director covers for the NP I would assume those visits are just billed under the doctor under normal circumstances. Having a locum cover your physician should make no difference in this scenario.

Just my take on it,
