If a radiologist decides to leave a practice and goes to work in a hospital, can the practice submit claims under the radiologists' NPI if a locum tenens covers the practice? What if the hospital is submitting claims for the radiologist at the same time the practice is submitting claims for the locum tenens?
 
I believe you have to file the claims using the NPI of the Locum Tenens with a modifier designating that they are temporary. Don't have my book at work with me but there is a modifier for this.

Locums get billed under the NPI of the doctor they are filling in for, with modifier Q6. I would say if one Dr. left the practice, and there is a temporary provider hired until a permanent replacement can be found, you can't bill under the NPI of the provider that left, as s/he is no longer employed there.
