We do the billing for Emergency Room physicians, and we have a locum tenens doctor now working with us. If the locum tenens doctor is working for an ER physician, and one of our Physician Assistant's see the patient. For billing purposes if it is a Medicare patient, the PA will sign the ER chart and the locum tenens doctor will sign the chart also. Will we still make the PA primary doctor on this charge unless the doctor has dictated a note and seen the patient? If he does see the patient we still need to add a modifier Q6, is this correct? We want to make sure before we bill any Medicare patient's.
