Wiki Locum Tenen billing for NP's and PA's

How is an NP and PA billed regarding locum tenen? Do you need to add a modifier and supervising provider to bill commerical and medicare and medicaid?

Has anyone used the U7 modifier for providers who are not yet credentialed but seeing patients?
