DESCRIPTION OF PROCEDURE:
There was a very small hiatal hernia. We dissected out the left side
of the esophagus at the diaphragm level, dissecting all the way down
to the crural pillar on the left side. We then dissected on the right
side of the esophagus opening up the gastrohepatic ligament, but
preserving the hepatic branch of the vagus nerve on the right side of
the esophagus. We then took down some of the fat along the anterior
gastroesophageal junction. We made a small window at the midportion
of the anterior border of the right crus of diaphragm, identified the
posterior vagus nerve. We made a window between the posterior vagus
nerve and the esophagus toward the left side of the esophagus. Once
we had a clear space and we are safe, we had our window and dissected
all the way to the left side of the esophagus. We placed a Penrose
drain through this tunnel, it was a retroesophageal tunnel, but in a
plane between the posterior vagus nerve and the esophagus.
We then placed our LINX device in the abdominal cavity. We tunneled it through the retroesophageal tunnel and secured it in place. We removed the
Penrose drain. We were happy with placement of the
device and with the placement, it did seem to reduce the small hiatal
hernia very nicely. We then removed our liver retractor and trocars
under direct vision.
