For me, if I already have 2 levels the same, I don't even bother leveling the third category.
This is not a problem my providers are treating/addressing, but I suppose I would put the problem as low in most circumstances. Perhaps being closest to an acute uncomplicated illness or injury? Depending on what diagnosis the provider is documenting, and the level of detail, MAYBE moderate. I suppose in extreme circumstances, it might even be high "acute/chronic injury that poses a threat to life" - like for a patient who is a sex worker or IV drug user not using clean supplies.
