Good Afternoon,

We are a DME company billing for DVT units across ASC centers in the US, we are constantly asked for LMNs.
What are the guidelines to follow for LMNs?
Specifically, can a doctor sign a "master" LMN w/ the necessary info, and we make copy of it for each of his patients, VS him actually signing it for each patient?
