Patient with documented mild recurrent major depressive disorder. Latest PHQ 9 score meets criteria for moderate recurrent MDD, however, at that visit, provider used existing diagnosis of "mild", increased medication management and documented increased symptomology without addressing the severity of illness in the chosen diagnosis code. The question is, would a query to change the diagnosis to Moderate recurrent MDD be considered a leading query where they assigned "mild" at that date of service? Or would a query be appropriate to address the specificity of the chosen diagnosis considering the increased symptomology and medication management change? Thank you in advance for your input!
 
My opinion is that it depends how the query is worded. If you say "Didn't you mean to state moderate since the PHQ score is 9?" would be leading. If you say "There is a discrepancy in the records. Please clarify the severity of MDD.", I would not consider that leading. You are clarifying as the records indicate 2 conflicting items in different areas.
