I have a procedure where a pt has a Pop artery thrombosis and MD first performed a thrombectomy, but no clot was removed, then went on to do a Artherectomy followed by a TPA infusion over 6 hrs. My question is, can the thrombectomy be coded (37184) or is it bundled with the other two procedures?

"Attention was made for recanalization of the right popliteal atery. Fro that, the 5-french sheath was changed to a 6-french Ansel 70 mm long sheath, which was placed into the right distal SFA. The total occlusion of the right popliteal atery was crossed with the help of 0.035 Trailblazer over an Advantage Glidewire. 5 mL of contrast wasf given and showed patency of the right AT proximally. Next, thrombectomy was perfromed of the right Pop artery with the help of the Trailblazer device. No significant clot was retrieved. At that point, further angiograms of the right POP artery showed that there maybe fan underlying residual plaguw, which was obvious after further dilatation with a 4mm x 80 mm balloon. Hence, decision was made to insert a distal protection device and excision atherectomy of the right popliteal artery was perfromed with a calcium cutter device"

He ended the procedure with a TPA infusion going....
