ANESTHESIA: Local anesthesia followed by 1% lidocaine was used.

PROCEDURES PERFORMED: Patient had an abdominal aortogram performed along
with bilateral runoff. Patient had selective right superficial femoral
artery angiogram and the runoff of the right lower extremity.

DESCRIPTION OF THE PROCEDURE PERFORMED: After patient was brought to the
angiographic suite, access was obtained in the left common femoral
artery using modified Seldinger method. The 5-French sheath was
inserted. After that, a 5-French Omniflush catheter was placed on an
0.035 Storq wire and brought in the distal aorta and runoff was perform
ed. After runoff was performed, the 5-French Omniflush sheath and the
0.035 wire was used to move the Omniflush sheath in up-and-over fashion
to the contralateral side and the catheter was brought in the proximal
right superficial femoral artery and rest of the imaging was performed
through that.

FINDINGS: On the right lower extremity, patient had a 70% deep venous
tract stenosis and patient had 90% stenosis of the posterior tibial
artery and two-vessel runoff. On the left lower extremity, patient's
superficial femoral artery had moderate disease with the rest of these
being around 40%-50% in the distal SFA. Patient also had three-vessel
runoff and moderate to diffuse disease of the below-knee arteries of the
left lower extremity.
