We've got a few people in the office that will send the physician the entire LCD informing them that "the dx that they used is not covered". Here is a list of the dx covered.

I think that this is totally inappropriate, correct?
 
I agree with you, this is not appropriate. There are many good articles available on the appropriate process for when and how to query a physician - here's a link to one here on this site:

As the article describes, questions should not be leading and should not be based on financial impact, and the generally accepted formats for querying a physicians are with open-ended, multiple choice or yes/no questions. An LCD and diagnosis code list might be a useful tool to share with a physician in an educational session, but to submit that with a query could definitely be construed as leading the physician.
