I am submitting a claim for a TTE which LCD-L27360 is for, but now I am not sure I am reading the LCD correctly. code 93320 is billed w/DX 413.9 on the LCD it is listed in group 1 so I looked under those DX's and 413.9 is there. But my claim keeps getting denied. Can anyone tell me if I am reading this right or if I need to look in another group on this LCD Thank you for you help. Nancy

You need to check the 2nd group of diagnosis codes listed and just above that group is the procedure code you are trying to bill - in the lcd the appropriate diagnosis are listed under the category with your procedure code and read any notation for that category and it also needs to meet medical necessity. Hope this helps
