Is there any way I can obtain a list for all ICD 9 codes that either consider medicaly necessary OR NOT Medical necessary for LCD L26884.
I have reveived a denied for OT CPT 90003,97110 & 97140 with ICD 9 as 780.79,V45.89 & 354.0 as not been medically necessary?
your input appreciated.

If this is OT for rehab you must use a V57 code first listed, this is in the coding guidelines under general guidelines, #15 encounters for rehab.
