Please help and advise if the layered closure is documented appropriately for billing. Thanks.
CPT for billing 11406, 12032
OP: An oblique incision was made in the anterior abdominal wall over the palpable lesion and it was removed and sent to pathology. It appeared consistent with a large bilobed keratinous or sebaceous cyst. It was removed and sent to pathology. Digital and visual inspection confirmed the lesion was completely removed. Hemostasis was observed with electrocautery. That wound was then closed similarly with interrupted deep dermal Vicryl suture and a running subcuticular Vicryl suture. The wound was dressed with dry gauze. The patient tolerated the procedure well and was transferred to the recovery area in satisfactory Condition.
Path reads
“abdominal wall mass”. The specimen consists of what appears to be a disrupted cyst measuring approximately 4.5 x 2.0 x 1.0 cm. The wall of the cyst is no greater than 0.2 cm. in thickness. Cross sections of the specimen reveal a large amount of yellow-tan keratinous substance.
