Can I use 27720 for lateral tibial plateau malunion fracture. The code does not specify, when i read the description and look at the picture it shows the shaft??? Any help or guidance is appreciated.

The code description does not specify only the tibial shaft but just lists repair of a nonunion or malunion of the tibia. I agree with CPT 27720 if a graft was not placed during the procedure.
CPT 27720: the physician makes an incision on the medial aspect of the tibia through the periosteum. The bone is divided by transverse cuts and the pieces are manually rotated into position and stabilized with a metal plate and screws.
