Hello, how would you bill for this procedure for a patient with Medicare?

The person who was coding the procedures and trained me said to use 64640 but I do not agree with that code as that is a destruction code (radiofrequency ablation of the lateral branches) and his note clearly states the a block.

I agree that if it is not a radiofrequency than 64640 would not be correct. You would have check with your Medicare carriers policy on use of the 76 modifier with nerve block codes versus modifier 51 or quantity.

CPT 64450 would seem that it be appropriate selection per individually block performed.
