If "late effects" may occur at anytime and a patient is 11-1/2 weeks post closed radial head fracture (813.05) and is now also diagnosed with medial epicondylitis (726.31) secondary to exercise, is it coded as a "late effect" (905.2)?
 
The thumb rule is we should only code late effects of something when it is documented by provider as such. We should not guess it by our virtue. Moreover, medial epicondylitis involves humerus epicondyle and they are anatomically separate in location within elbow.
