Hello,
Do we code both the V15.3 Hx of Radiation and the 909.x code for the Late Effect of Radiation? They don't exclude each other, but it seems redundant.

Coding Late Effects

In most instances, two codes are required to completely code a late effect. The first code sequenced identifies the residual conition that is currently affecting the patient.
The second code listed describes the cause of the late effect-the original illness or injury that is not longer present in the acute form. Got to 'Late Effect' in the index (905,906,907.x)
Exceptions:
The health record does not identify the residula effect.
ICD9 does not provide a code to describe the cause of the late effect.
The alphabetic index index indicates a different sequence.

The late effect code can be assigned by itself it eh diagnostic statement indicating a late effect does not include the residual condition
