Wiki Laparoscopic Cholecystectomy

I've run into this scenario a couple times in the last couple of weeks. A laparoscopic cholecystectomy w cholangiography and laparoscopic exploration of the common bile duct. I find 47562 cholecystectomy only, 47563 w the cholangiography, and 47564 w the bile duct exploration. I don't see any laparoscopic codes for all 3 together, or cholangiography or bile duct exploration alone. How do you code this scenario?

Don't know if this will help but the Ingenix coding companion states
The gallbladder is removed in a laparoscopical surgical session (47562). Any method of removal may be employed. Report 47563 when a contrats agent is injected into the cystic duct and an image taken in order to identify stones or strictures
Report 47564 when the common bile duct is explored in the course of the laparoscopic session
