I'm in need of some guidance!
My surgeon's OR report showing that he performed everything for the vaginal hyst but when they tried to remove the uterus they could not due to the sm size of the pelvic cavity & the large uterus. (The fibroid & ovaries were removed vaginally.) So then they did the abdominal approach to remove the uterus.
My head is telling me to code just the TAH but once I read the OR report I am not certain. The surgeon documentated really well for both of the procedures, it was a 6 hr surgery.
