Wiki Lap Band Adjustment

Does anyone know if there is certain criteria to be met in order to charge for a lap band adjustment in the office? Is it a requirement for billing that the amount of fluid taken out or put in and the patient's reaction afterward be documented?

Just record it like a procedure note. There are no "requirements" that need to be met that I have found so far.
