Wiki Lap and Open Procedures

Pt was diagnosed with gallbladder issues and had a CT where a colon mass was found.

She was taken to surgery and had a lap chole, and the doc then mobilized the splenic and hepatic flexures, but the colon mass was too big to resect laparoscopically. He converted to open and took the mass "en bloc", taking with it a section of large and small bowel. He anastamosed the large bowel back together, and then the small bowel.

Not sure how to code this...I cant submit a laparoscopic mobilization of the splenic flexure with an open partial colectomy....

Any ideas???

Use 44139. The mobilization codes are add-ons and they say to add 44213 to 44204-44208, and add 44139 to 44140-44147.

 

[colorectal surgeon]

44120 for the small bowel resection
44140 for the colon resection
v64 .41 for laparoscopic procedure converted to open

I would probably just add the 22 on to the colectomy

Maybe someone can explain what 49205 means and could this apply to this case?
