Hello everyone,

I am seeking help with coding for a herniated nucleus pulposus with lumbar disk disease L5-S1.

I'm not sure that I have correctly choosen the proper laminectomy codes.

brief over view of Operative Procedure

Laminotomy was created at L4 on right -side to confirm adqueate positioning of pedicle screw. (63030) O-arm intraoperative CT scanning as well as Brain Lab navigation system was utilized for placement of instrumentation and interbody graft...

A central posterior decompression was accomplished at L5-S1 with radical facetectomies and formainotomies created. (63047, 63048)...

The big difference is the purpose of the procedure: You should report 63030 when laminotomy is performed with a diskectomy, to treat spinal disc herniation using either an open procedure or under endoscopic assistance.

The code was revised starting in 2000 to this use, as explained in the November 1999 CPT Assistant:

"Code 63030 has been revised to clarify the appropriate reporting of this code for use when performing laminotomy for diskectomy using open and endoscopic assistance."
