We have some discrepancies in our office in regards to how dx codes should be attached to visits. Currently our providers fill out a lab order sheet and then we have instructed the lab to send out labs with dx provided by the provider.

The lab is finding that there are times where a dx is not written for the test. Can the lab review the chart to find a dx? There are repeat tests that are being done every few months for the same reason (ex. high risk meds).

Lab personnel assigning dxs

Technically lab personnel do not assign dxs. They are to use what is on the order. Appears to be a provider education opportunity.
