Would this be 63047 + 63048? Dr. coded 63012, 63005, 63047 and 63048
I am confusing myself with 63047 again. Does a lami, facetectomy, and foraminotomy have to all be preformed?

A midine incision was made overlying the L4 & L5 spinous process are and carried down through the sub tissue with a Weitlaner retractor inserted.
The Bovie electrocautery was used to divide the musculature and fascia free from the spinous processes and lamina of L4 and L5 as well as a transitional spinous processes and lamina of L4 & L5 as well as a transitional level that would otherwise be termed the L6 and S1 Level.
A Collis retractore was inserted and bilatearl lami carried out at L4 and L5 levels. At the
L4-5 interspace, the stenosis was quite severe due to hypertrophied ligamentum flavum, facet hypertrophy, and spondylolisthesis. Foraminotomy was carried out at the L4-5 adn L5-S1 levels bilaterally with Kerrison punch rongeurs till the nerve roots were free in the interspinal space.
