With E/M and getting your MDM points, and with a ureter and kidney stone being a combination code-N20.2, do you count two diagnoses or just one?

Wiki Kidney/ureter stone -N20.2. One or two for MDM?

So, I'm thinking it will all depend on the plan of care. If the actual 'MDM' supports one plan of action for both stones, then 1 point.
But if different approaches and such will have to be taken to treat each stone, then 2.
