I have a family practice doc who billed 20610 with J3301. BCBS of SC is denying the J code as part of the injection. No office visit was coded with this visit. I know that BCBS of SC has paid this in the past, but has denied the last several this doc has done. I tried and appeal but original decision upheld. I can not find anything that states this should be bundled. I asked BCBS why this might be bundled, and was referred to an education specialist.
 
We process thru BC of CT and our 20610, J3301 always get paid. Are you putting a RT/LT modifier on 20610 and attaching the DRG number for the J3301 on the HCFA?
