My office is in Ohio and we use J3490 for bupivicaine, lidocaine, & propofol and others as needed. Can you code for bupivicaine when used with depo medrol? We also code with the NDC, dosage, etc. Some insurances pay, but Medicare does not pay for the bupivicaine or lidocaine. They will pay for propofol. We also were told by CGS, our new Medicare carrier, to input the NDC, dosage info, name of the drug, amounts purchased all with equal signs. However our clearing house software states that there are invalid characters in the claim and if we use more than one J3490 not all of the NDC info fits in box 19. Any suggestions?

In my office we had the same problem with our clearinghouse. So, we just put the name and dosage on the 1st line. Usually we get an ADR anyway. I'm in FL so we get ADR letters for almost everything. That's when I send the invoice. I've been told by my drug rep that the clearinghouse can change this so all the info goes over. I have yet to get that to happen. :eek: Sorry
