Has anyone had any rejections on J3301 for service description? We haven't had this issue until November so I'm not sure what's changed.

Does your claim specify Kenalog and does it have the NDC#?

We have the NDC # but not Kenalog. We've never had to enter that before so that's what's confusing me.

Is it only one payer or plan or all claims that have this? What does the clearinghouse say?
