Below are the cpt codes I am billing for a heart cath. Can anyone help me with acceptable modifiers for BCBS?

BCBS keeps denying 36010 saying it is inclusive of another code.

I'm not sure what all was done or from what access but the work to insert a catheter into the superior/inferior vena cava are included in the work to insert a catheter into any of the selected blood vessels described in procedure codes 36011-36015.

36011 is inclusive to 36010. 36010 is bundled into 36011.
