Lately, we had lot of denials from Medicare when billing IV iron as not coverage under patient dx. For years, we always bill with 280.9 and 995.20. Recently, i called medicare, they stated Medicare only pay for IV iron when pt has either chronic kidney disease. Has anyone run into this problem.

We usually use 280.8 or 280.9 stand alone and have never had an issue. My guess is the adverse effect code is the hang-up, not sure why that code would be attached to an IV iron infusion.

There could also have been a change with your carrier in regard to dx requirements, but they should have it posted on their LCD if there was.
