Clinician saw patient from 11:32 am to 1:40 pm

Do you bill as 90837 with 11:32 am to 1:40 pm and then have 99354 without any times noted? or do you split the times?

If you split the times, would you use 90837 - 11:32am to 12:32pm and 99354 12:33pm to 1:40pm
or
90837 - 11:32 to 1:01 and 99354 - 1:02-1:40

you go by total time spent.
you have documented 128 minutes.
for this scenario the first 60 minutes goes to the 90837, subtract that from total time gives you 68 minutes left. to use the 99354 you must have a minimum of 30 minutes left (this is why it states a 90 minute minimum encounter.) the 99354 will account for additional time of 30 thru 74 minutes, you have 68 so you may add the 99354.
